Why is my multifactor authentication not prompting?

If you have multifactor authentication enabled and it is not prompting, if you are logging into the offline cached file - this is expected. LastPass cannot prompt for a multifactor for the local login (with the exception of Yubikey and Sesame). 

If you are not logging in offline, then this is likely because

The device is set as trusted. To untrust your device, do the following:

  1. Go to your LastPass Vault
  2. Click on Settings 
  3. Click the 'Trusted Computers' tab
  4. Disable or delete the appropriate devices.

 

or 

You are logging in offline. Multifactor Authentication requires you to be online in order to authenticate. If you want LastPass to prompt you for your multifactor every time, offline mode must be disabled. To disable offline mode, do the following:

  1. Go to your LastPass Vault
  2. Click on Settings
  3. Click the 'Multifactor Options' tab
  4. Choose the multifactor option you use
  5. Toggle 'Permit Offline Access' to 'Disallow'
